TREATMENT & CONSENT

You hereby request and give consent to our practitioners to perform all necessary examinations, mobilisations, therapy, rehabilitation and medical diagnostic procedures in accordance with their professional training and understand of your presenting issue. You understand that during your treatment, care may be rendered by different practitioners at Performance Medicine.

You understand that you have an opportunity to discuss with your practitioner the nature and purpose of your care before any treatment is rendered.

You acknowledge and accept the following:

– The physical nature of physiotherapy, myotherapy, remedial massage, and related treatments.

– You have the right to see the practitioner of your choice, refuse intervention, or seek further opinion, and to provide feedback or make a complaint.

– The adverse risk associated with treatment may include stiffness & soreness (musculoskeletal), neurological complications, cerebrovascular injuries, skin irritations, as explained to you by your therapist in accordance with best practice.

– The probability of risk is lowered by screening & assessment procedures.

– Other treatment options exist if the risk of physiotherapy, myotherapy, or remedial massage is considered to be too high, including telehealth, medication, medical care, hospitalisation, and surgery.

– The risk of remaining untreated may include musculoskeletal issues such as scar tissue and other degenerative changes, these may impact musculoskeletal health and perpetuate the complexity of a condition and its rehabilitation.

REFERRALS

Most patients can receive our services without the need of a medical or other referral. However, in the event that the patient is referred from another pracitioner, it is recommended that a brief written referral is provided by the treating therapist to assist in the continuity of care and our delivery of the most appropriate services.

ACCOUNTS & BILLING (PRIVATE BILLING)

It is expected all fees are paid in full at the time of consultation.

We accept cash, EFTPOS, credit card, direct deposit, HICAPS & medipass. In the event of electronic network failure, full payment is still expected on the day of consultation.

Payment of all accounts can be made via telephone or in person.       

Health insurance rebates (electronic or otherwise) can only be claimed after the service has been rendered. In the instance of purchasing a Class Pass, rebates will be available once the class has been attended or once the pass has been used in entirety.

Those who feel they cannot comply with our billing methods or wish to be given special consideration, please feel welcome to put your request in writing and address to the Practice Operations Manager.

Please understand our billing methods allow equality of service to all clients, and serve to raise the already high standard of health care we can offer you.

LATE NOTICE FEE POLICY (CONSULTATIONS)

It is expected that if a patient cannot keep an appointment with us, that notice is given as soon as possible by calling the practice (and leaving a message if after hours).

If less that 24 hours notice is given, a $55 fee will apply. However, provision of another appointment within business hours on the day of original appointment will see the fee waived.

If a Monday appointment, you must notify us by 2pm on the prior Saturday to avoid a fee of $55.

In the event a late cancellation is received, and the patient does not accept our offer of an alternative appointment within business hours of the same day, payment of the full $55 fee is expected within 48 hours.

These policies related to all patients irrespective of their method of funding. In the case of compensable patients (worker’s compensation, transport accident, etc.), cancellations and missed appointments must be paid privately by the patient and cannot be charged to the paying party.

LATE FEES (CONSULTATIONS)

Patients arriving late to an appointment, but still within their booked time may receive a shorter consultation in an effort to respect the time of other waiting patients.

Patients who either arrive after or miss entirely an appointment will be charged the full price.

LATE NOTICE & MISSED (CLASSES)

It is expected that if a patient cannot keep a class booking, that notice is given as soon as possible by calling the practice (and leaving a message if after hours).

If less that 24 hours notice is given, a $20 fee will apply, or a class will be deducted from their prepurchased Class Pass. However, provision of another class attended within business hours on the day of original class will see the fee waived.

If cancelling a Monday class, you must notify us by 2pm on the prior Saturday to avoid the above fee.

In the event a late cancellation is received, and the patient does not accept our offer of an alternative class within business hours of the same day, payment of the full $20 fee is expected within 48 hours.

Patients who either arrive after or miss entirely a class will be charged in line with the above cancellation policy.

WORKCOVER PROCEDURE

Patients who believe they are eligible for workcover must ensure they satisfy the following criteria before requesting billing to a compensable body:

– The injury occurred directly in relation to their current employment.

– The injury has been reported to the employer and all necessary internal paperwork has been completed.

– A workcover claim form has been submitted to the employer, a claim acceptance letter has been received and presented to Performance Medicine, and where required a letter of approval from the employer is provided.

– The patient signs the necessary forms to notify the appropriate bodies of the Treatment Plan.

– There may be times when a workcover appointment is required for the purpose of filling in documentation as required by the compensable body.

ACCOUNTS & BILLING – WORKCOVER

– The individual patient is ultimately responsible for payment of all accounts at the time of treatment. All billing is in line with our current fee schedule.

– On receipt of payment, an invoice will be issued for the claimant to lodge with their workcover insurance or employer for approved reimbursement.

– All administration fees for missed appointments, late cancellations, or overdue accounts will be charged directly to the patient.

– In line with workcover policy, Performance Medicine will complete and provide the patient with all relevant documentation, reporting and correspondence with the relevant parties.

– Perofrmance Medicine reserves the right to discontinue treatment through workcover if we feel a claimant is acting either fraudulently or demonstrating non-compliance.

TAC PROCEDURE

Patients who believe they are eligible for Transport Accident Commision (TAC) funding must ensure they satisfy the following criteria before requesting funding to TAC:

– The injury occurred directly in relation to a motor vehicle accident.

– The injury has been reported to the police and all necessary paperwork has been completed.

– A claim form has been submitted to the TAC, a claim acceptance letter has been received and presented to Performance Medicine, and where required a letter of recomendation from a GP is provided as per TAC guidelines.

– The patient signs the necessary forms to notify the appropriate bodies of the Treatment Plan.

– There may be times when a TAC appointment is required for the purpose of filling in documentation as required by the TAC.

– In line with TAC policy, Performance Medicine will complete and provide the patient with all relevant documentation, reporting and correspondence with the relevant parties.

– Perofrmance Medicine reserves the right to discontinue treatment through TAC if we feel a claimant is acting either fraudulently or demonstrating non-compliance.

PRIVACY POLICY

The information you provide during your consultations will be collected for the purpose of providing high quality health care. This practice is committed to protecting your privacy and this information is generally only disclosed to other members of your medical team where necessary. It will however be disclosed to other organisations where required by law or if necessary for debt recovery purposes. You may gain access to information about you held by this clinic by putting your request in writing and attentioning it to the Practice Director.

INFORMATION RELEASE

To expedite the management of your injury and or condition, you accept and consent that it may be necessary to communicate details of your medical condition with treating practitioners, consultants, case managers, and employers. By agreeing and consenting to this section, you give permission for representatives of Performance Medicine to exchange such information regarding injuries sustained in the aforementioned incident and any significant past medical history with those people deemed necessary by Performance Medicine.
